Turns out just like EVERY entity of business I've dealt with. Dishonorable, says one thing and does another or does not have the information needed to effectively communicate about forms needed to return to work. Also, does not have form for injuries that are not workers compensation form, so will ask you to request one from your employer (What doctor does that?). We had nearly an hour discussion of my injuries and how they made it difficult to perform my day-to-day duties, agreed that would be difficult and advised would have me 'Return to Work' note the at next appointment but didn't have form. I came back when he said he would have it available, then he changed it to. If I knew you needed this form, we could've discussed this in the beginning. We talked for an hour before my first session started. I wouldn't recommend somebody that's not 100% trustworthy. I recommend getting permission to recorder or have a third-party present to account for conversation. This lack of accountability will cause me more problems.
